Athletic Participation Forms – Aktivate

Students interested in participating in school athletics must complete all required eligibility forms through our online athletic management platform, Aktivate.
Before a student may attend tryouts or participate in any athletic activities, all required documents including a current sports physical and any additional athletic forms must be completed, signed, and uploaded to Aktivate. Students who have incomplete or missing documentation will not be permitted to try out until all requirements have been met.
Families are encouraged to complete the registration process as early as possible to allow time for document review and approval before tryouts begin. It is encouraged to go ahead and make an account and begin uploading documents even if the student is not planning to tryout for a sport until the winter or spring season.
